Submit only pages 1 & 2 with payment.

Checks Payable to “CoRe Publications” in US.

Dollars, drawn on a US Bank.

Prices subject to change without notice.

Incorrectly tabulated orders will result in quantity adjustments of materials shipped.

Incorrect shipping address/zip code will result in additional shipping/handling charges.

Order discrepancies must be reported within 30 days of receipt of order.

All Sales Final.
