Quarterly Service Report (QSR)
 

CoDA is looking for information to put in the QSR from the CoDA committees.  The Report comes out each quarter and CoDA would like to see each committee include something in each report.

NOTICE:  The Board voted in their 7 Nov 09 face-to-face to have the QSR published for a true quarter.  Thus,

Deadlines:  Below are the deadlines for the committees to submit something for inclusion in that quarter's report:

1st Qtr:  Apr 15th

2nd Qtr:  Jul 15th

3rd Qtr:  Oct 15th

4th Qtr:  Jan 15th

Submission:  Each committee should prepare the report and once it is ready to submit, send it to qsr@coda.org with a carbon copy to qsrliaison@coda.org.

Publish dates:  QSR editor will strive to send the report out within 30 days of receiving all submissions.

Mailing:  The report is emailed or mailed upon your request when it is created.  If you do not currently receive a copy, you can sign up at www.codependents.org  or use the form below to receive a mailed copy.

Posting:  The reports are posted very soon after publication and archived at www.coda.org/status/index.php.
